If you’re not an existing customer, please mail or fax your catalog request on your dance school’s business letterhead along with one of the following:
  • Voided business deposit slip
  • Voided business check
  • Newspaper advertisement — must include business name and address imprinted on it
  • Yellow pages ad — must include business name and address imprinted on it
  • Business license
